Occupational Therapist (OT) Citrus Heights Post Acute
Responsibilities
- Evaluate residents within 24 hours (or one business day) of physician referral
- Develop individualized treatment plans and obtain physician approval
- Provide evidence-based occupational therapy treatments to maximize independence and function
- Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely
- Collaborate with physicians, nursing staff, families, and the interdisciplinary care team
- Participate in care conferences, rehabilitation meetings, and discharge planning
- Train restorative nursing staff and caregivers on maintenance programs and safe patient care techniques
- Supervise Certified Occupational Therapy Assistants (COTAs), rehabilitation aides, and therapy students in accordance with state practice guidelines
- Recommend adaptive equipment and durable medical equipment (DME) to improve resident safety and independence
- Participate in facility education and staff in-service programs
- Maintain therapy equipment and report maintenance concerns promptly
Qualifications
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program
- Current California Occupational Therapist license in good standing
- New graduates are encouraged to apply
- Skilled nursing or post-acute experience preferred, but not required
- Skilled in communication, organization, and clinical reasoning
- Experience with PointClickCare, Casamba, or other electronic medical record systems preferred
- Ability to work collaboratively within an interdisciplinary team
- Passion for delivering compassionate, resident-centered care
