Occupational Therapist (OT) Citrus Heights Post Acute

Responsibilities

  • Evaluate residents within 24 hours (or one business day) of physician referral
  • Develop individualized treatment plans and obtain physician approval
  • Provide evidence-based occupational therapy treatments to maximize independence and function
  • Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely
  • Collaborate with physicians, nursing staff, families, and the interdisciplinary care team
  • Participate in care conferences, rehabilitation meetings, and discharge planning
  • Train restorative nursing staff and caregivers on maintenance programs and safe patient care techniques
  • Supervise Certified Occupational Therapy Assistants (COTAs), rehabilitation aides, and therapy students in accordance with state practice guidelines
  • Recommend adaptive equipment and durable medical equipment (DME) to improve resident safety and independence
  • Participate in facility education and staff in-service programs
  • Maintain therapy equipment and report maintenance concerns promptly

Qualifications

  • Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program
  • Current California Occupational Therapist license in good standing
  • New graduates are encouraged to apply
  • Skilled nursing or post-acute experience preferred, but not required
  • Skilled in communication, organization, and clinical reasoning
  • Experience with PointClickCare, Casamba, or other electronic medical record systems preferred
  • Ability to work collaboratively within an interdisciplinary team
  • Passion for delivering compassionate, resident-centered care
