Responsibilities

• Perform occupational therapy evaluations in patients’ homes.
• Develop individualized treatment plans based on physician orders and patient needs.
• Collaborate with an interdisciplinary healthcare team to deliver exceptional patient care.
• Complete timely and accurate clinical documentation.
• Support the continued growth and development of the skilled home‑health program.

Requirements

• Current Occupational Therapist license in the State of New Mexico.
• Graduate of an accredited Occupational Therapy program.
• Home‑health experience preferred but not required.
• Strong clinical judgment and excellent communication skills.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Ability to work independently while delivering compassionate, patient‑centered care.
