We're fixing home healthcare, and we're building our presence in Detroit. Adaptive is an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We're building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.

Objectives

  • Carry a high-quality caseload
  • Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores
  • Participate in case conferences/team huddles to improve our quality
  • Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent
  • Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows

Must-haves

  • Active Michigan OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management

Benefits

The rest of the package includes medical/dental/vision benefits, paid time off, and a compensation range of $100K - $130K for full-time positions.
