Occupational Therapist (OT) Home Health - Part Time

About the role

ABL Health Care and ABL Hospice are dedicated to delivering compassionate, personalized care for patients and their families through home healthcare and hospice services. We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Part Time (Per Diem) Occupational Therapist (OT) in Oakland.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administers treatments, monitors patient conditions, and reports signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current OT License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver's License
• Able to travel within a 15 - 25 mile radius on a regular basis.
