Occupational Therapist (OT) - Home Health

Responsibilities

• Provide skilled occupational therapy to patients in their homes, helping them recover, manage chronic conditions, and maintain independence.
• Conduct patient assessments, establish therapy goals, document progress, and assist with exercises to improve motor skills and coordination.
• Recommend adaptive equipment and strategies to enhance home safety.
• Educate patients and families on care plans and self-care.
• Assess patient needs, observe deficits, develop and implement therapy plans, and promote patient safety and independence.
• Support personalized patient care through dedicated, compassionate service.

Qualifications & Skills

• Active Occupational Therapy license.
• Successful completion of the NBCOT certification exam.
• Ability to work collaboratively within a home health team, focusing on quality patient outcomes.
