Occupational Therapist (OT) - Home Health

About the Role

We are seeking a Home Care Occupational Therapist to evaluate and develop treatment plans for patients of all ages, focusing on enhancing daily living activities.

Responsibilities

• Review and communicate care plans with physicians
• Educate patients and families on treatment and safety
• Document visits, assessments, and progress updates
• Set and revise patient goals
• Coordinate case conferencing and discharge planning
• Order necessary supplies for patient care
• Conduct comprehensive evaluations and home safety assessments
• Develop, adjust, and implement individualized care plans
• Instruct patients and caregivers on therapeutic techniques
• Ensure thorough documentation of all interactions and interventions

Supervision & Collaboration

• Supervise Occupational Therapy Assistants and Home Health Aides
• Provide instructions and ensure compliance with home care regulations
• Work effectively within interdisciplinary teams

Requirements

• Bachelor’s degree in Occupational Therapy
• Licensed Occupational Therapist in Illinois
• BLS certification
• Valid driver’s license and auto insurance
• Ability to assess patient safety and educate caregivers
