Occupational Therapist | (OT) - Full-Time or Part-Time

Location: Lancaster & Mineral Point, WI – Lancaster Health Services and Mineral Point Health Services. Setting: Skilled Nursing Facility (SNF). Flexible PRN opportunities also available.

About the role

Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team. This full-time or part-time opportunity offers the chance to provide exceptional occupational therapy services across supportive, collaborative skilled nursing facilities where your clinical expertise makes a meaningful impact every day.

As an Occupational Therapist, you will work with adult and geriatric patients to improve independence, safety, mobility, and quality of life through individualized occupational therapy interventions.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a skilled nursing facility setting.
• Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness.
• Help patients improve independence and participate in meaningful daily activities.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Deliver compassionate, evidence-based occupational therapy services.

Requirements

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered care.
