Occupational Therapist (OT) for Home Health

About the role

FeldCare Connects is seeking a self-motivated Occupational Therapist to serve the Ojai area. The ideal candidate will have a strong background in Occupational Therapy and a passion for delivering premier excellence of care to adults of all ages.

Responsibilities

• Assess and evaluate patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment
• Determine a treatment plan based on assessment and evaluation
• Perform interventions aimed at improving and enhancing patient's wellbeing
• Evaluate patient's progress
• Provide services defined by state laws governing the practice of Occupational Therapy, in coordination with other members of the health care team
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety
• Observe, record, and report the patient's response to treatment and changes to the patient's condition
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient

Qualifications

• Occupational Therapist license and registration by the state
• Completion of an accredited Occupational Therapist program
• Ability and enthusiasm in working with all ages
• Bilingual a plus
