Occupational Therapist (OT) for Home Health

About the role

FeldCare Connects is seeking an Independent Contractor Occupational Therapist to serve Stockton.

Responsibilities

• Assess and evaluate patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
• Determine a treatment plan based on the assessment and evaluation.
• Achieve and enhance patient wellbeing through interventions.
• Evaluate patient progress.

Requirements

• Provide services according to state laws governing Occupational Therapy.
• Lead patients in therapeutic, creative, and self-care activities.
• Observe, record, and report patient responses and condition changes.
• Instruct patients, families, caregivers, and the health care team on therapy areas.

Qualifications

• Ability and enthusiasm in working with all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Bilingual a plus!
