Occupational Therapist (OT)

Home Health Occupational Therapist (OT) Provide personalized occupational therapy to patients in their homes, helping them regain independence and perform daily activities. Collaborate with a multidisciplinary team, assess patient needs, set therapy goals, and document progress. Assist with exercises, recommend adaptive equipment, assess fall risks, and educate patients and families on care plans. Supervise and guide Certified Occupational Therapy Assistants. Qualifications Include Graduate of an accredited OT program and active OT license in ColoradoAt least one year of home healthcare experienceCPR certification, background check, and drug screeningKnowledge of home health regulations, clinical judgment, and strong interpersonal skills Key Duties Deliver therapy focused on improving motor skills and home safetyAssess patient deficits and establish individualized goalsEducate patients and families on therapy and safety strategiesSupport team collaboration to optimize patient outcomes Ideal candidates are compassionate, resourceful, and dedicated to making a meaningful impact through home-based therapy.
