About the role

Watertown Health Care Center is a skilled nursing home facility seeking an Occupational Therapist to join our team. We take pride in providing our residents with an amazing experience through a tight-knit family of caregivers who serve with compassion, care, and a positive attitude. Our primary goal is to help residents return to independence while maintaining a friendly environment and providing nutritionally enhanced meals.

Responsibilities

  • Evaluate and document evaluations of referred residents in accordance with facility policies and procedures
  • Provide appropriate treatment to help residents maintain or attain the highest level of function possible within the confines of their physical abilities
  • Recommend and/or provide appropriate treatment, preventative measures, and adaptive equipment to prevent decline in residents' function
  • Provide appropriate resident or caregiver education to help residents maintain or attain the highest level of function possible
  • Maintain timely and accurate resident reports as required by federal, state, and local regulatory agencies and third-party payers
  • Make recommendations to administration for adaptive equipment and preventative measures
  • Observe infection control procedures
  • Be responsible for the safety of residents under supervision
  • Assume duties related to completion of Minimum Data Set assessments as assigned
  • Complete documentation related to evaluations, treatment plans, MDS assessments, resident care plans, and progress notes in a timely manner
  • Participate in resident care conferences, interdisciplinary care conferences, and rehabilitation conferences as scheduled
  • Participate in implementation of programs to improve resident care related to Quality Indicators
  • Assume Quality Assurance duties as assigned
  • Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants, and ancillary service providers
  • Observe all facility policies and procedures, safety policies, and guidelines in the employees' handbook
  • Follow facility Residents' Rights policies and adhere to Corporate Compliance Program Code of Conduct
  • Protect the privacy of resident Protected Health Information
  • Perform all other duties as assigned

Qualifications

  • MS in Occupational Therapy
  • NBCOT Certified
  • Excellent verbal and written communication and organizational skills
  • Ability to read, analyze, and interpret common medical information
  • Ability to effectively communicate with residents, their families, and other healthcare professionals
  • Ability to present information to supervisor
  • Ability to define problems, collect data, establish facts, and draw conclusions
  • Familiarity with software such as Word and Excel
