Occupational Therapist (OT) — Assisted Living Facility

About the role

Occupational therapy in the assisted living setting focuses on helping people regain the activities that define their lives. At TheraDynamics, our OTs work with patients recovering from strokes, hip fractures, cardiac events, and progressive neurological conditions. You will design creative, functional interventions addressing ADLs, cognitive retraining, upper extremity rehabilitation, and safe discharge planning. Your work directly impacts whether a patient returns home or remains in a facility, and we provide the tools, time, and support to make that happen.

Responsibilities

• Conduct comprehensive occupational therapy evaluations.
• Develop individualized treatment plans focused on functional independence.
• Collaborate with nursing, PT, SLP, and social work to ensure coordinated patient care.
• Address self-care skills, cognitive and perceptual deficits, home safety, adaptive equipment needs, and caregiver training.
• Apply clinical reasoning in a supportive environment with realistic productivity expectations.

Requirements

• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Current or eligible for New York State OT license.
• NBCOT certification (or eligible).
• Passion for helping patients regain independence.
• Strong clinical reasoning and creative problem-solving skills.
