About the Role

Home Sweet Home is seeking a compassionate Occupational Therapist (OT) to join our growing Home Health team. As a leading provider of skilled Home Health services throughout Maricopa County, we help patients regain independence and improve their quality of life through personalized, one-on-one care in the comfort of their homes.

Responsibilities

• Complete in-home Occupational Therapy evaluations and reassessments
• Develop individualized treatment plans to improve independence with Activities of Daily Living (ADLs)
• Provide rehabilitation for patients recovering from illness, injury, surgery, or hospitalization
• Recommend adaptive equipment, home modifications, and fall prevention strategies
• Educate patients and caregivers on safety, mobility, and functional independence
• Collaborate with physicians, nurses, Physical Therapists (PT), Speech-Language Pathologists (SLP), and the interdisciplinary care team
• Document patient progress accurately using EMR while maintaining Medicare and agency compliance

Requirements

• Current Arizona Occupational Therapist (OT) license
• Graduate of an ACOTE-accredited Occupational Therapy program
• Minimum one year of Occupational Therapy experience (Home Health preferred)
• Strong communication, organization, and time management skills
• Experience with EMR documentation
• Valid driver's license, reliable transportation, and current auto insurance
