Your Work Environment
You’ll be part of the therapy-specific team supporting Home and Heart Home Health Care Agency, working within a collaborative, multidisciplinary model. Care is coordinated, patient-centered, and team-driven, with shared accountability and open communication.

Your Patients
You’ll work with a diverse caseload, including a higher-than-average post-operative orthopedic population.

Position Requirements
- Graduate (current or pending) of an accredited occupational therapy program
- Active or pending state occupational therapist license
- New graduates encouraged to apply
