Occupational Therapist, Home Health

About the role

We are seeking a Home Health Occupational Therapist to provide personalized rehabilitation care in patients' homes.

Responsibilities

• Assess patients' daily living skills and functional abilities
• Develop and implement individualized therapeutic programs
• Collaborate with healthcare teams to coordinate patient care
• Document care progress and update treatment plans
• Administer standardized tests and evaluations
• Guide patients through treatment plans and therapeutic exercises
• Recommend and train patients on adaptive equipment use
• Train family members and caregivers on patient support techniques
• Participate in discharge planning and community reintegration activities
• Ensure comprehensive patient support throughout the care continuum

Requirements

• Accredited degree in Occupational Therapy
• Current state licensure as an Occupational Therapist
• CPR certification
• Reliable transportation and valid driver’s license
• Strong organizational and communication skills
• Experience in home health preferred
