Occupational Therapist, Home Health

About the role

We are seeking a Home Health Occupational Therapist to provide personalized rehabilitation care in patients' homes, helping them regain independence and improve their quality of life.

Responsibilities

• Assess patients' daily living and work skills.
• Develop and implement individualized therapeutic programs.
• Collaborate with healthcare teams to coordinate patient care.
• Instruct patients and families on treatment plans and home exercises.
• Review and document patient progress regularly.
• Design community reintegration activities to support patient recovery.
• Recommend and train patients on adaptive equipment.
• Supervise therapy staff, including assistants and aides.
• Maintain accurate documentation and communicate effectively with physicians and team members.

Requirements

• Degree from an accredited Occupational Therapy Program.
• Current and unrestricted Occupational Therapist license.
• CPR certification.
• Reliable transportation and a valid driver’s license.

Preferred Qualifications

• At least six months of Occupational Therapy practice experience.
• Prior home health experience.
