Job Details

• Location: Easton, MD (Talbot, Dorchester & Caroline Counties)
• Setting: Home Health
• Schedule: Monday-Friday | 8:00 AM - 4:30 PM
• Hours: 40 Hours/Week
• Productivity: 30 Points/Week
• Mileage Reimbursement: Yes (between patient visits)
• On-Call/Weekends: None

Pay

• $53 to $57/hr on W2

Responsibilities

• Conduct Home Health OASIS assessments, evaluations, reassessments, routine visits, and discharge visits
• Develop and implement individualized treatment plans
• Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Educate patients and caregivers to improve independence and quality of life

Required Qualifications

• Active Maryland Occupational Therapist (OT) License
• Home Health experience required
• OASIS documentation experience required
• Valid Driver's License and reliable transportation
