Role Overview As a Home Health Occupational Therapist, you will deliver personalized rehabilitation care in patients' homes to enhance their strength, mobility, and independence. Your responsibilities include assessing patients' daily living and work skills, developing and adjusting therapeutic programs, collaborating with healthcare teams, and providing treatment based on physician orders. You will oversee patient progress, participate in discharge planning, design community reintegration activities, and recommend adaptive equipment. Supervising and instructing therapy assistants and family members, documenting care, and ensuring compliance with guidelines are key duties.
