About the role

The Occupational Therapist is responsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients. Responsible to supervise Certified Occupational Therapy Assistants (COTA) and cosign their documentation when required. Responsibilities

• Provides skilled occupational therapy in accordance with the authorized practitioner's plan of care.
• Treats the patient through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function.
• Demonstrates and teaches alternate techniques to complete activities of daily living, proper transfers, positioning and all other OT functions as stated in their practice act.
• Accepts only patients referred by the Team Supervisor, Case Placement Coordinator or designee.
• Demonstrates competency in skills required by the agency on at least an annual basis at the time of field evaluation.
• Evaluates and assesses the patient's level of function by applying diagnostic and prognostic procedures and tests and assists in the development and revision of the plan of care.
• Assesses the patient's continual care needs.
• Ensures that the plan of care addresses all problems identified in the assessment or documents rationale for not doing so.
• Identifies specific DME/orthotic needs of patient and refers to appropriate vendor.
• Observes, records and reports the patient's reaction to treatment and any changes in the patient's condition to the RN or RN Coordinator or Team Supervisor.
• Reports routinely every 30 days
