About the role
Provide therapeutic services in home care settings to help patients improve their functional abilities and quality of life.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Implement and modify treatment plans based on patient progress and feedback
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Document patient progress and communicate with referring physicians and other healthcare providers
Requirements
- Bachelor's degree in Occupational Therapy from an accredited program
- Current New Jersey license as an Occupational Therapist
- Experience in home care setting preferred
