Responsibilities
- Screens, evaluates, re-evaluates and treats patients referred by a licensed provider.
- Captures and communicates treatment activities with health care team members and the patient.
- Works closely with the interdisciplinary team to ensure high quality patient care.
- Evaluates the patient's need for occupational therapy and develops/carries out appropriate treatment plan.
- Instructs family members, COTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
- Communicates with the physician and other health care providers on a regular basis regarding the patient's progress.
- Provides clinical oversight of COTAs in the home.
Requirements
Must be a licensed Occupational Therapist in Vermont and New Hampshire or hold a compact license. Graduate of an accredited school in Occupational Therapy. One year of experience as an OT in a home health care setting is preferred. Unencumbered OT license in VT and NH or compact.
