About the role
Help patients improve their ability to safely perform everyday activities while promoting independence in the home environment.
Responsibilities
- Deliver occupational therapy services in patients' homes
- Complete initial evaluations and follow-up visits
- Perform fall risk assessments and home safety evaluations
- Develop and implement individualized treatment plans (OT)
- Auxiliary with treatment interventions established by the supervising OT (COTA)
- Educate patients and caregivers on adaptive techniques and equipment
- Maintain patient progress and communicate changes with the interdisciplinary team
- Complete accurate and timely clinical documentation
Requirements
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Home health experience preferred but not required
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
Qualifications
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team

Pay
- Occupational Therapist (OT): $80 per routine visit
- Certified Occupational Therapy Assistant (COTA): $60 per visit
