Key Responsibilities
- Provides program participant evaluations, occupational therapy services, and may supervise Certified Occupational Therapist Assistants.
- Affirms individuals across all populations and develops effective, individualized treatment plans.
- Provides therapy services, updates treatment plans, and communicates patient needs with team members.
- Manages caseloads, re-evaluates progress, and adjusts programs as needed.
- Conducts home evaluations and establishes home exercise programs.
- Maintains accurate and timely documentation, including reports and scheduling.
- Collaborates with medical professionals, staff, families, and community resources.
Specialty & Program Areas
- Assists with Custom Wheelchairs (PT/OT collaboration)
- Assists with Custom Durable Medical Equipment (DME) (OT/PT collaboration)
- Supports for Feeding Services (High-Need Area) in collaboration with OT/SLP
- Delivers Community-Based Services (marketed as community services, not home health) across all disciplines
- Delivers Telehealth Services across all disciplines, as needed
- Travels as needed
Qualifications
- Master’s Degree in Occupational Therapy with at least one year of experience in human services.
- Experience in assistive technology evaluations, preferred.
- Current licensure as an OT in the state of Arizona, and in good standing with AOTA.
- Valid driver’s license, car registration, and auto insurance.
- Strong communication and interpersonal skills.
- Reliable, responsible, and compassionate approach.
