Experience
Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members. Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures. Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
Responsibilities

- Establishes and updates formal plan of care to address individual patient needs.
