About the role
We are seeking a licensed Occupational Therapist to provide personalized, one-on-one care to older adults in private residences and senior living communities.
Responsibilities
- Perform evaluations, develop individualized care plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
- Document clinical activities accurately within electronic medical records while adhering to regulatory standards.
- Collaborate with an interdisciplinary team of professionals including PTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth and virtual care to enhance access and continuity of care.
Qualifications
- Valid occupational therapy license in the practice state or eligibility.
- Degree from an accredited OT program.
- Proficiency in computers and EMR systems.
- Current CPR certification.
Physical Requirements
- Exert medium physical effort; assist with patient transfers; perform therapeutic activities; and drive to patient locations.
- Ability to wear PPE and perform CPR when necessary.
