About the role

Occupational Therapist providing in-home, senior living, and virtual care to older adults.

Responsibilities

• Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life
• Provide one-on-one therapy in various settings including private residences and senior communities
• Document sessions in EMR, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth and virtual care to enhance access and flexibility

Qualifications & Skills

• Valid OT license
• Degree from an accredited program
• Basic EMR proficiency
• CPR certification
• Physical ability to perform in-home care tasks
