About the Role
Provide personalized in-home and senior living care to older adults, focusing on promoting mobility, independence, and wellness through evidence-based occupational therapy.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Deliver evidence-based therapy sessions to enhance patient outcomes.
- Perform documentation within EMR systems, ensuring compliance with regulatory standards.
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to support safe aging in place.
- Incorporate telehealth and virtual care to enhance access and flexibility.
Requirements
- Valid Occupational Therapy license or eligibility to obtain one.
- Degree from an accredited OT program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
Physical Requirements
- Ability to perform moderate physical tasks, including lifting and repositioning patients.
- Standing, walking, climbing stairs, and driving to patient locations.
