Occupational Therapist

About the Role

We are seeking licensed Occupational Therapists to provide personalized, one-on-one care to older adults in their homes or senior living communities, utilizing our Geriatric House Calls™ model. The role involves conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy sessions focused on enhancing mobility, independence, and overall quality of life. Virtual care options and flexible scheduling are integral to the position, supporting continuity of care and work-life balance.

Responsibilities

• Provide in-home and community-based OT services
• Perform assessments and develop care plans
• Administer therapy to improve mobility and independence
• Document care activities accurately in electronic medical records
• Collaborate with interdisciplinary teams
• Educate patients, caregivers, and staff on safety and functional aging
• Utilize telehealth when appropriate

Requirements

• Valid Occupational Therapy license in applicable state(s)
• Degree from an accredited OT program
• Proficiency in computers and EMR systems
• Current CPR certification
• Ability to exert medium physical effort, including lifting and repositioning patients
• Perform CPR and assist with patient transfers
• Drive to patient locations and wear PPE as needed
