Occupational Therapist

About the role

The Occupational Therapist provides clinical occupational therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and instructing participants, caregivers, and staff on maximizing safety and functional independence in Activities of Daily Living (ADLs), exercise programs, and DME use. As a core member of the Interdisciplinary Team (IDT), this role contributes clinical assessments and care coordination in a collaborative spirit that informs whole-person care planning, while maintaining documentation that meets all regulatory requirements. Beyond direct care, the Occupational Therapist serves as a subject matter expert for occupational therapy program standardization, team education, and quality audits, supports DME inventory management, and upholds safety and infection control standards — all while contributing to a center culture grounded in Habitat's values and participant belonging.

Responsibilities

• Contribute to a center experience that participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.
• Provide occupational therapy evaluation and treatment on an acute, restorative, and maintenance level to program participants.
• Participate in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
• Assess condition, create a treatment plan, and discuss implementation of therapy with participants and caregivers.
• Make home visits as needed and document activities including participant/caregiver response in participant’s chart.
• Educate participants and family members in proper care and use of various supports, exercise programs, and DME.
• Document therapy services provided and perform department record-keeping within documentation requirements.
• Assist in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
• Maintain quality assurance, safety, environmental, and infection control programs.
• Perform data collection, prepare reports, and maintain necessary participant statistics.
• Attend all staff meetings and patient care conferences as scheduled.

Requirements

• Master's or Doctorate in Occupational Therapy (MOT or DOT) or equivalent from an accredited program meeting AOTA & ACOTE standards.
• National Certification by the American Occupational Therapy Association or eligible for the National Registration Examination of the American Occupational Therapy Association.
• A minimum of one year of experience working with the frail or elderly.

Preferred Qualifications

• 2+ years as a practicing OT.
• Bilingual: Spanish/English strongly preferred; additional/other languages a plus.
• PACE experience and/or ADCH/CBAS experience.
• A state-issued driver’s license, personal transportation, and auto insurance as required by law.

Pay

The anticipated base salary range for this position is $112,000 - $129,000. This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
