About the role

Provide personalized, one-on-one therapy to older adults in private homes and senior living communities, enhancing mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop individualized care plans.
• Deliver evidence-based treatments and document care in EMR systems.
• Collaborate with interdisciplinary teams and educate patients, caregivers, and staff to promote safety and aging in place.
• Incorporate telehealth and virtual care options to increase flexibility and patient access.

Requirements

• Valid OT license.
• Basic computer skills and CPR certification.
• Physical capability to perform in-home care tasks, including patient transfers and mobility activities.
