Occupational Therapist

Responsibilities

• Assess referred patients requiring rehabilitation services
• Provide direct patient care to assess medical condition, functional capabilities, limitations, and restrictions and potential for rehabilitation
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician
• Set realistic and achievable goals for patients
• Document and record patients' condition
• Educate patients and families in appropriate therapy methods
• Evaluate patients, maintain weekly progress notes, prepare re-certification documents, and manage discharges
• Collaborate with all disciplines to provide comprehensive patient-centered care
• Comply with confidentiality, HIPAA, and federal residents' rights

Qualifications and Skills

• Graduated from an accredited Occupational Therapy college or university
• Valid professional license and registration for state of practice
• Experience with long-term care and/or geriatric care preferred but not required
