Responsibilities
- Provide occupational therapy services to adult and geriatric patients in their homes
- Complete comprehensive evaluations of patients' functional abilities, including: Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), Strength, endurance, coordination, balance, and mobility, Cognitive, psychological, sensory, and motor function
- Develop individualized treatment plans designed to maximize independence and safety
- Audit home environments and recommend adaptive equipment and environmental modifications
- Implement therapeutic interventions that improve function and quality of life
- Collaborate with nursing and the interdisciplinary care team to coordinate patient care
- Educate patients, caregivers, and family members on therapeutic techniques, home safety, and strategies to promote independence
- Reassess patient progress and modify treatment plans as appropriate
- Complete accurate and timely electronic documentation using an EMR system
Qualifications
- Current Occupational Therapist (OT) license in the State of New Jersey
- Minimum of one year of recent Occupational Therapy experience
- Home Health experience preferred, but not required
- Graduate of an accredited Occupational Therapy program
- Strong clinical assessment and critical thinking skills
- Excellent communication, organization, and time management abilities
- Comfortable using electronic medical records (EMR)
- Ability to work independently while collaborating with an interdisciplinary team
- Reliable transportation and willingness to travel throughout Monmouth, Middlesex, Somerset, and Union Counties
Pay
Base Pay, depending upon qualifications, status, and subject to negotiation: Full-Time $70.00–$75.00 per point ($109,200- $117,000 annual earning potential)
