Evaluations and Planning

This position is responsible for evaluating patient's functional abilities and limitations, establishing a plan of care and treatment goals, collaborating with patient and family/caregiver, initiating discharge planning, recommending additions to or modifications of referring orders, implementing individualized treatment plan, assessing ongoing effectiveness of treatment, modifying plan of care as indicated, providing skilled treatment in a safe manner, providing ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team, documenting according to standards set by the department and regulatory agencies, communicating evaluation/treatment results and patient progress to physicians, nurses, patient, family/caregiver, co-workers and the interdisciplinary team, defending treatment rationale with standard practices and/or current literature.
