Occupational Therapist

About the role

Occupational Therapist responsible for providing personalized, in-home and community-based therapy to older adults, supporting mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy sessions supporting patient wellness, mobility, and functional independence through one-on-one care in private residences and senior living communities.
• Document care using electronic medical records and ensure compliance with regulatory standards.
• Collaborate with interdisciplinary teams including physical therapists, speech-language pathologists, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Incorporate telehealth and virtual care to enhance access and flexibility.

Qualifications & Skills

• Valid Occupational Therapy license or eligibility in the state of practice.
• Degree from an accredited Occupational Therapy program.
• Proficiency in basic computer and electronic medical record systems.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning patients, and driving.
