About the Role

Deliver patient-centered occupational therapy services in the home setting to adults and seniors within a supportive, innovative environment.

Responsibilities

• Conduct functional evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to enhance independence, safety, and quality of life.
• Recommend home modifications and adaptive equipment to improve functionality.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Document care activities in the EMR system.
• Educate patients and caregivers to promote understanding and engagement.
• Contribute to quality improvement initiatives.

Requirements

• Licensed Occupational Therapist or eligible for licensure.
• Strong assessment and treatment planning skills.
• Excellent communication and teamwork abilities.
• Knowledge of home modifications and adaptive equipment.
• Commitment to patient-centered care and continuous improvement.
