Occupational Therapist

About the role

We are seeking passionate licensed Occupational Therapists to deliver personalized in-home and senior living care, enhancing mobility, independence, and quality of life for older adults using the Geriatric House Calls™ model.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Provide evidence-based occupational therapy interventions
• Document patient progress and treatment in electronic medical records (EMR)
• Collaborate with interdisciplinary teams to coordinate care
• Educate patients, caregivers, and staff on therapeutic techniques and safety
• Utilize telehealth opportunities where applicable

Requirements

• Valid Occupational Therapist license
• Degree from an accredited occupational therapy program
• Basic computer skills for documentation and communication
• Current CPR certification

Physical demands

• Assist with patient transfers and mobility
• Drive to patient locations for in-home visits
