Occupational Therapist

Seeking a licensed Occupational Therapist to provide personalized, one-on-one care to older adults in private homes and senior living communities. Responsibilities include conducting assessments, developing individualized care plans, delivering evidence-based treatments to enhance mobility, independence, and wellness, and documenting within the EMR system. Collaborate with a multidisciplinary team of PTs, SLPs, and fitness specialists, and educate patients, caregivers, and staff on safety and aging in place. Opportunities for telehealth and virtual care are available to increase flexibility and patient access. Qualifications include a valid OT license, degree from an accredited program, basic computer skills, and CPR certification. Physical ability to assist patients and perform in-home care tasks is required.
