Occupational Therapist

Role Overview: An Occupational Therapist providing personalized, in-home, and virtual care to older adults in private residences and senior living communities. Responsibilities: Conduct evaluations, develop care plans, deliver evidence-based therapy to enhance mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.Qualifications & Skills: Valid OT license, degree from an accredited program, proficiency in EMR and basic computer skills, CPR certification, and ability to meet physical demands including lifting and assisting patients.Key Duties: Support patient wellness through therapy, promote aging in place, and maintain effective documentation while building strong patient relationships.
