About the role
Provide personalized one-on-one occupational therapy to older adults in private homes, senior living, and community settings.
Responsibilities
- Conduct evaluations, develop individualized care plans, and deliver evidence-based treatments to enhance mobility, independence, and wellness.
- Document patient progress in electronic medical records, collaborating with a multidisciplinary team including PTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging-in-place strategies.
- Utilize telehealth and virtual care options to extend access and flexibility.
Requirements
- Valid occupational therapy license or eligibility in the state(s) of practice.
- Degree from an accredited OT program.
- Proficiency with basic computer and EMR systems.
- Current CPR certification.
- Ability to perform moderate physical effort, including lifting, repositioning patients, and extended walking or standing.
- Capability to provide assistance with patient transfers and perform therapeutic activities.
- Driving to patient locations and wearing PPE when necessary.
