About the role

Adaptive Home Health is building a higher-acuity, patient-centered home health model across Denver. Our mission is to dramatically improve patient access to home health care. As a Certified Occupational Therapy Assistant (COTA), you will be on the front line delivering high‑quality functional visits in the home under the OT plan of care, helping patients stay safe and independent. We develop technology to support field clinicians — our charting tools reduce documentation burden and our upcoming scheduling product will streamline visit coordination, reduce last‑minute changes, and protect clinician time.

Responsibilities

  • Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
  • Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
  • Track objective functional progress and communicate updates/concerns to the supervising OT.
  • Coordinate with nursing and the broader care team: escalate changes in condition and safety concerns.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

  • Must‑haves
    • Active Denver COTA license (in good standing)
    • Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)
    • Strong communication and documentation
    • Current driver’s license and comfort with daily travel across assigned territory
  • Nice‑to‑haves
    • Prior home health experience
    • HCHB experience

Schedule

Full-time Monday–Friday + occasional weekends (as needed).
