About the role

Evaluate patients with physical, emotional, cognitive, and/or psychosocial deficits.
Design and implement patient-centered, effective treatment plans using appropriate modalities.
Assess the patient's ability to safely perform independent living skills.
Document patient progress, monitor, and modify the plan of care as necessary.

Responsibilities

• Minimum weekend and holiday requirement.

Requirements

• Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Occupational Therapy (OT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.

Qualifications

• Bachelor's degree required.
• Master's OR Doctorate degree preferred.
