About the role
To assess the nutritional status of prenatal patients and develop a plan of care that will meet their nutritional needs and result in an optimal pregnancy outcome.
Responsibilities
- Certify the patient in WIC if eligibility criteria are met.
- Document the assessment and care plan in the Crossroads system.
- Document the plan and patient’s response to the plan in Crossroads and provide feedback.
- Assign food packages as needed to WIC clients.
- Assess the nutritional status of all maternity clinic prenatal patients by collecting and evaluating anthropometric, biochemical, clinical, dietary, and eco-social data.
- Develop an individualized nutrition care plan based on the assessment findings and assist each patient in understanding and implementing the plan through counseling, discussion, audiovisual tools, and printed educational materials.
- Provide ongoing follow-up to monitor the patient’s progress, ensure compliance, and adjust the care plan as needed.
- Reassess and modify the plan as the patient’s condition or needs change.
- Document all assessments, care plans, interventions, and patient responses in the electronic medical record.
- Develop therapeutic nutrition care plans for nutritionally high-risk prenatal patients.
- Maintain ongoing communication with the interdisciplinary team, including physicians, nurse practitioners, nurses, and social workers.
- Present updated nutrition information that may be applied in maternal care.
- In the event of a disaster, assist as needed.
Requirements
- Registered Dietitian with the Commission on Dietetics Registration; or Master's degree in Dietetics, Public Health Nutrition, or Nutrition; or graduation from a Commission on Accreditation for Dietetics Education - approved Didactic Program in Dietetics and two years nutrition experience;
- Dietetic Technician, Registered with the Commission on Dietetics Registration with a Bachelor's degree in any subject area from an accredited four year college or university and two years nutrition experience;
- A Bachelor's of Science degree in Dietetics, Public Health Nutrition or Community Nutrition from an accredited four year college or university and two years nutrition experience.
Qualifications
- Specialized knowledge in principles and application of nutrition and dietetics to improve health outcomes.
- Knowledge of relevant medical terminology and skill in documenting in the medical record.
- A wide based knowledge of nutrition, especially developmental and prenatal nutrition is necessary.
