Primary Responsibilities
- Meet with each resident on a regular basis to provide health and wellness- checks and answer any health-related questions.
- Review all resident information and complete an assessment of the resident prior to move-in, at move in, when changes in the resident’s condition occur, and as required by residence policy and state regulation.
- Order medications and oversee the resident’s medication schedule for residents receiving medication administration services.
- Ensure proper documentation for medication assistance/administration
- Develop and maintain an up-to-date individual service plan for each resident.
- Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs.
- Cookordinate additional health services the resident may need.
- Aid in coordinating resident transfer to and from a hospital or another facility.
- Aid in counseling residents and families.
- Be on call for medical questions and resident emergencies as scheduled.
- Aid in coordinating residents’ dietary needs with the Nutrition Coordinator.
- Review changes in residents’ condition with the Executive Director, and resident’s physician/s
- Assess residents and monitor their health.
- Ensure that all infection control guidelines are met.
- Maintain residents’ wellness files.
Training and Regulatory Compliance
- Facilitate OSHA training and assist employees with OSHA compliance.
- Oversee and assist with training regarding residents’ health and wellness.
- Cookordinate state-required training with Executive Director to ensure regulatory compliance.
Communications
- Attend all regular staff meetings and required training sessions.
- Attends various community events.
- Participates in the Manager On Duty program.
- Effectively communicate information pertaining to the residents on a “need to know” basis to other team members according to the residence policies and procedures.
- Notify your supervisor or the Clinical Director if you observe a change in a resident’s condition.
Qualifications
- Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required.
- One year experience working as a nurse in a hospital, long term care facility, or other related health care facility required.
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet preferred.
- Proficiency with electronic medical records (EMR) preferred.
