Responsibilities

  • Assist patients with daily tasks like bathing, dressing, and using the bathroom
  • Help patients move safely, including transfers and using wheelchairs, walkers, or canes
  • Provide nursing care by performing medical tasks including vital signs, weight, finger-stick blood tests, ECGs, and breathalyzer tests. Collect specimens for point-of-care testing as needed.
  • Make sure patients eat, drink, and stay hydrated
  • Assist with room searches, skin assessments, conduct patient safety checks are required
  • Guide patients for scheduled medications, provider visits, lab tests, groups and activities, etc.
  • Maintain a safe and caring environment, delivering care in a culturally sensitive manner and consistent with Wellbridge mission, vision, and values
  • Monitor medical supply inventory and replenish stock
  • Provide assistance during admissions, transfers, and discharges
  • Provide emotional support and help during emergencies and/or crisis situations. Complete incident reports as necessary
  • Encourage patients to take part in their treatment and recovery

Essential Functions

  • Ability to assess medical needs and provide patient care. For patient care needs, this is an on-site role.
  • Consistently move self about patient care areas.
  • Occasionally lift and move medical equipment and supplies up to 50 pounds without assistance.
  • Ability to verbally communicate related to patient care.
  • Consistent computer and phone, general office equipment use.
  • Update medical records while in a stationary position up to 30% of the time.

Qualifications

  • High school diploma or equivalent
  • Certified Nursing Assistant (CNA) license in NYS preferred.
  • Related experience in healthcare, nursing, or behavioral health. Experience in detox, rehab, or withdrawal management settings is preferred.
  • Basic Life Support (BLS) certification (or obtain within 60 days)
  • Basic knowledge of substance use and mental health required, familiarity with OASAS, ASAM, and Joint Commission standards is preferred.
  • Effective communication and teamwork skills

Pay

This position is eligible for applicable shift pay differentials.

Schedule

  • Day shift: 7a-7:30p
  • Night shift: 7p-7:30a
