Responsibilities

• Deliver patient care including personal care (ADL): bathing, body mechanics, nutrition, elimination, safety, special procedures, housekeeping services, and participation in interdisciplinary team conferences.
• For NAs 1+4: Perform sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; insertion of urinary catheter (replacement only); oxygen therapy including room setup and monitoring of flow-rate.
• For NAs II: Break-up and remove fecal impaction; assist with I.V. fluid-assisted activities; nutritional activities; suctioning; tracheostomy care; insertion of urinary catheter (replacement only); sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; oxygen therapy including room setup and monitoring of flow-rate.
• Attend and participate in interdisciplinary team conferences (IDT) and special case conferences.
• Establish and maintain effective working relationships with team members and identify specific patient/caregiver needs, making appropriate referrals.
• Achieve and maintain accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure/certification standards.
• Demonstrate competency in knowledge and skills, maintain professional reputation, engage in professional development, and meet current licensure, CPR certification, and CEU requirements.
• Maintain productive and professional relationships with external agencies and healthcare providers and exhibit time management skills.
• Clean patient rooms as required per patient room cleaning procedure, ensuring all areas are clean and stocked with supplies, keeping family rooms clean throughout the day, and using proper chemicals and protective equipment when working with chemicals.

Requirements

Minimum qualifications: 1 year of experience required. Experience in home care, working with terminally ill, and/or geriatrics is preferred.

Qualifications

Must possess a valid state nursing license and be certified as a Hospice and Palliative Nurses (CHPN).
