Position Summary

The Registered Nurse (RN) is responsible for overseeing and coordinating all aspects of medical care for individuals receiving services through our programs. Working as part of a multidisciplinary team, the RN provides nursing care from a holistic, recovery-oriented, and values-based perspective. The RN supports individuals by promoting physical and behavioral health, coordinating medical services, managing medications, providing health education, and advocating for integrated healthcare solutions that enhance overall wellness and quality of life.

Essential Responsibilities

  • Clinical Care and Health Monitoring

  • Assess, monitor, and respond to new, ongoing, and emerging health concerns.

  • Conduct health screenings and assessments utilizing evidence-based and validated tools.

  • Monitor vital signs, health conditions, medication effectiveness, and potential side effects.

  • Assess medical care needs and recommend health-related goals and interventions to support recovery and wellness.

  • Administer medications and treatments, including intramuscular (IM) injections, in accordance with physician orders and nursing standards.

  • Medication Management

  • Oversee the ordering, delivery, storage, administration, and documentation of medications.

  • Ensure compliance with organizational policies, nursing regulations, and medication management standards.

  • Collaborate with individuals and treatment teams to improve medication adherence and treatment outcomes.

  • Maintain accurate medication records and monitor for medication-related concerns.

  • Care Coordination

  • Coordinate healthcare services with primary care providers, physicians, specialists, prescribers, pharmacies, and other healthcare professionals.

  • Serve as a liaison between individuals, healthcare providers, families, and community supports.

  • Assist individuals in accessing medical care and community resources that promote integrated physical and behavioral health.

  • Support efforts to ensure individuals receive annual physical examinations, dental care, preventive screenings, and recommended follow-up services.

  • Recovery-Oriented Services

  • Deliver nursing care that supports recovery-focused, person-centered, and evidence-based treatment approaches.

  • Participate in the development and implementation of individualized recovery plans.

  • Support individuals in achieving health, wellness, and recovery goals.

  • Promote dignity, independence, self-determination, and overall quality of life.

  • Education and Training

  • Provide health education and wellness information to individuals receiving services.

  • Offer guidance, consultation, and training to staff regarding health concerns, treatment protocols, and best practices.

  • Encourage informed decision-making and active participation in care.

  • Documentation and Compliance

  • Complete assessments, progress notes, treatment contacts, recovery plans, and other clinical documentation accurately and within required timeframes.

  • Maintain compliance with organizational, licensing, accreditation, and funding requirements.

  • Ensure documentation reflects current health status, treatment interventions, and outcomes.

  • Utilize collaborative documentation practices to engage individuals in the treatment process.

  • Crisis Intervention

  • Participate in a rotating on-call schedule to provide emergency and crisis intervention support.

  • Respond effectively to urgent medical and behavioral health situations.

  • Coordinate care during emergencies and communicate appropriately with treatment teams and healthcare providers.

  • Cultural Responsiveness

  • Provide culturally competent and individualized services that respect and value each person's unique background, identity, experiences, and needs.

  • Foster an inclusive environment that promotes equitable and respectful care for all individuals served.

  • Additional Duties

  • Meet required service delivery and treatment contact expectations.

  • Participate in interdisciplinary team meetings and case reviews.

  • Perform additional duties as assigned by the Assistant Program Director or designated supervisor.

Qualifications

  • Graduate of an accredited professional nursing program (Associate Degree, Diploma Nursing Program, or Bachelor of Science in Nursing).

  • Current and unrestricted Registered Nurse (RN) license.

  • Strong clinical assessment, nursing, and care coordination skills.

  • Ability to work effectively within a multidisciplinary team environment.

  • Excellent communication, documentation, and organizational skills.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).

  • Experience in behavioral health, community health, substance use treatment, psychiatric rehabilitation, or integrated healthcare settings.

  • Knowledge of recovery-oriented and trauma-informed care practices.

  • Exert up to 50 pounds of force occasionally

  • Use hands and fingers to operate equipment and perform nursing tasks

  • Communicate effectively through speaking and hearing

  • Visually assess individuals, documentation, and surroundings

  • Push and pull equipment or supplies

  • Perform repetitive motions

  • Reach above and below shoulder level

  • Kneel, stoop, crouch, and crawl as necessary
