Nurse Program Coordinator: Headache clinic and Neurology infusion center

About the role

Full Time Days, variable shifts 40 hours/week; prefer experience with IV's and medports. Summary: at least one (1) year of leadership experience preferred. The Nurse Program Coordinator is responsible for assessment, coordination of care, discharge planning, nursing care delivery, evaluation, and documentation of care for pediatric patients and their families.

Responsibilities

• Assesses patient/families physical, emotional, spiritual, psychosocial and learning needs; communicates pertinent information to the team and other health care providers; assess and recognize changes in patient condition and family dynamics.
• Provides care coordination and care planning for target population
• Provides nursing care to target population of patients and documents evaluation and care delivery in patient plan of care.
• Determines the education needs of nursing personnel, multi-disciplinary team members and others; implements patient and family education in collaboration with the multi-disciplinary team.
• Provides leadership to the nursing team and acts as a change agent to improve patient care.
• Serves as a resource to other nurses and healthcare providers with expertise in assigned practice area.
• Other duties as required.

Requirements

• Education: Bachelor's degree required
• Certification: RN Licensure required
• Years of relevant experience: 3 – 5 years required

Skills

• Experience in pediatrics required
• Experience in communication, teaching skills, and problem-solving skills required
• Experience working with all levels within an organization preferred
• Experience in healthcare required
• Proficiency in MS Office [Outlook, Excel, Word] or similar software preferred
• Epic software preferred
• Prefer experience with IV's and medports
• At least one (1) year of leadership experience preferred
