About the Role

TruHealth is seeking an advanced practice provider—either a Nurse Practitioner (NP) or Physician Assistant (PA)—with experience in primary care or internal medicine, preferably with a geriatrics focus. This position offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. Our providers work primarily within nursing homes or assisted living communities, caring for residents enrolled in special Medicare Advantage plans. You will provide personalized, coordinated healthcare that enhances quality of life and reduces unnecessary hospital visits. As an advanced practice provider, you will exercise independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician as needed. You will also work closely with facility partners and health plan leadership to implement our national model of care in compliance with all federal and state regulations.

Responsibilities

  • Assess patients’ medical and mental health needs and develop proactive care plans that manage conditions and address barriers to care.
  • Ensure compliance with local, state, and federal regulations related to clinical services provided.
  • Prescribe medications, order lab work, diagnostic procedures, and consultations.
  • Monitor patients’ compliance and response to treatment, modifying plans as necessary.
  • Collaborate with RN case managers as part of an integrated care team.
  • May provide training and advice to facility staff.

Requirements

  • FNP, AGNP, AHACNP, or PA license required.
  • Degree from an accredited APRN or PA program.
  • Minimum of 3 years’ experience in clinical nursing or rehabilitation within geriatric populations.
  • Experience with Electronic Health Records (EHR).
  • Working knowledge of Microsoft applications, including Word, Outlook, and Excel.
