Nurse Practitioner - Urgent Care/Primary Care

About the role

We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our growing team in a hybrid role providing both Primary Care and Urgent Care services.

Responsibilities

• Provide comprehensive primary care services for pediatric (if applicable), adult, and geriatric patients.
• Perform annual wellness visits, preventive screenings, and routine physical examinations.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Develop individualized treatment plans and provide patient education.
• Order, interpret, and communicate laboratory and diagnostic test results.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with physicians, specialists, nursing staff, and care management teams to ensure coordinated patient care.
• Coordinate specialty referrals and follow-up care.
• Participate in quality improvement initiatives and population health programs.
• Ensure compliance with organizational policies, evidence-based clinical guidelines, and regulatory requirements.
• Document in the electronic medical record (eClinicalWorks preferred).
• Communicate effectively with patients and families on disease prevention, wellness, medications, and treatment plans.

Requirements

• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Current New York State Nurse Practitioner license.
• National Board Certification (AANP or ANCC).
• Current DEA registration and New York State Controlled Substance Registration.
• Minimum of 2 years of Nurse Practitioner experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Emergency Medicine.
• Current BLS certification.
• Strong clinical assessment, diagnostic, and procedural skills.
• Excellent communication and patient education abilities.

Preferred

• Experience with eClinicalWorks (eCW).
• Bilingual (Spanish or another language) preferred.
• Experience treating patients across all age groups.
• ACLS certification.
