About the role

The Primary Care NP will be responsible for continually assessing and retooling clinical workflows in conjunction with GRMC in order to maintain effective, efficient practice. The Lead NP position also serves on site and network level committees including the joint primary care/operations committee and the primary care team leaders committee in order to represent the concerns of their fellow NPs at the site level, and bring input into decision-making regarding policy development, standardization of network protocols, and other issues influencing the experience of clinicians at South City Hospital.

Responsibilities

  • Maintain Clinical Oversight, and assure that the clinical setting is functional and equipped for daily patient care.
  • Work with clinical staff and Medical Director (including Front Desk, Medical Assistants, and Nursing, Nurse Practitioners) to maintain or exceed expected weekly / monthly productivity.
  • Ongoing monitoring and adjustment of processes responsible for smooth and effective patient flow.
  • Provide clinical leadership and mentorship for Nursing and Nurse Practitioner practice
  • Share new clinical policies / procedures
  • Promote Quality Improvement processes, power cycles, and support network improvement initiatives and quality measure goals.
  • Facilitate morning huddles and assist in running clinical staff meetings with focus on primary care team issues and functioning.
  • Encourage and promote teamwork. Nurture individual team member’s strengths and abilities.
  • In collaboration with the RN, oversee Medical Assistant functions including: ordering medical supplies, vaccines, inventory, clinical office needs, and daily patient flow.
  • Work with the Medical Director, House Supervisors, and other staff to cover unexpected clinical absences. Assist with coverage issues as needed.
  • Report staff concerns to the Medical Director.
  • Conduct routine supervision meetings, at least twice a month, with Director of Primary Care and Operations Site Director to facilitate smooth communication and promote excellence in patient care services offered on-site.
  • Resource and liaison for other members of healthcare team (Behavioral Health, Outreach, Social Work etc.) Work in conjunction with Center Director to promote positive team dynamics through regular meetings and staff supervision.
  • Hold regular supervision meetings with nurse practitioners and RNs, promote productivity, identify strengths and develop individualized plans to address areas of improvement or development, or desired acquisition of skills. Supported by Primary Care Director.
  • Create and maintain work schedule of Nurse Practitioners to ensure 24/7 coverage.
  • Report clinical staff concerns to the Medical Director.
  • Input into disciplinary action regarding clinical staff. Initiate process when indicated. Consistent and timely documentation and communication of problems regarding primary care staff.
  • Aid in interviewing and hiring of primary care clinical staff in collaboration with Medical Director and Chief Medical Officer.
  • Network with other Primary Care Site Directors; meet as needed to ensure standardization of clinical policies and procedures between sites.
  • Contribute to the revision and approval of policies, protocols, and network clinical guidelines.
  • Attend meetings / conferences regarding clinical management or other topics that help promote clinical practice.
  • Participate in continuing education activities that will enhance management and administrative knowledge and skills and contribute to professional growth and leadership abilities.
  • Perform other duties as assigned.

Requirements

  • Minimum 5 years as a Nurse Practitioner / Demonstration of clinical competence
  • Graduation from Associate, Diploma, or Bachelor degree Nursing Program.
  • Must have knowledge of Growth and Development and show clinical expertise for ages of patients served.
  • Licensure as a Nurse Practitioner in the State of Illinois.

Skills and Abilities

  • Completion of CPR.
  • Evaluative skills necessary for patient assessment.
  • Decision making skills required for treatment planning/implementation.
  • Clinical reasoning and judgment skills necessary for evaluation of nursing interventions and for safety of patients and self.
  • Excellent oral and written communication skills to interact with patients, visitors and staff. Ability to recognize need for and alter communication with patients with communication impairments.
  • Ability to manage time, organize daily schedule to meet productivity standards, and cope with professional stress.
  • Completion of clinical training and competency in stroke care.

Physical Requirements/Working Conditions

  • Ability to stand and walk for long periods of time
  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
  • Ability to transfer and move patients who vary with ability to assist weighing 200+ lbs. using proper lifting techniques and with assistance as necessary
  • Ability to lift objects and to push and pull equipment weighing in excess of 40 lbs. using proper techniques and with assistance as necessary
  • Manual dexterity, coordination and skillful use of hands when working with patient and equipment, which may include pushing buttons, adjusting dials, turning on/off switches, buckling, holding and positioning limbs/body parts, etc.
  • Ability to physically perform necessary documentation in writing or use of tape recorder
  • Sufficiently sighted, including color discrimination in designated departments, to perform analyses
